feat(provider): add Command Code presets and quota support (#7358)

- add Command Code presets for Claude Code and Codex
- map the Claude and Codex provider endpoints and model catalogs
- detect both base URLs and query credits, rolling limits, and subscription periods
- document the preset and usage coverage in English, Chinese, and Japanese
- add preset routing and quota parsing tests
