Jason e12415b5ba refactor(providers): move OpenCode/OpenClaw set-default into the more menu ...

The icon-only star on added OpenCode/OpenClaw cards was hard to read and
had no selected state. "Set as default" now sits at the top of the card's
more menu, like the aggregation page; providers with several models open
a submenu to pick the default model. The card keeps only the remove
button, and the primary button no longer needs its own dropdown.
