zhayujieandcowagent 91b47119f7 fix(claude): default request max_tokens to 64K for Claude 4+ ...

Only the older Claude 3.x line has a small output cap; default to 64000 so a
newly released model (e.g. a future claude-*-6) no longer silently regresses
to the 8192 fallback — no per-model code change needed.
